Summary of Changes
We have made the following changes to Form SSA-L5061:
On the cover letter, we have deleted the third paragraph that begins “The Social Security Administration…” since this information the Privacy Act Statement includes this information.
We revised the Privacy Act Statement as recommended by SSA’s Office of General Counsel.
We will issue old stock until exhausted and then we will use the revised version.
