Revisions to the Collection Instrument:
SSA is revising the Internet form General Request for Social Security Records (eFOIA) in order to update the look and feel of the form and add an explanation of when it is better to send a paper request, or when it is inappropriate to use it. We are also updating the screens to provide SSA’s mailing address for requests for information other than FOIA requests. SSA is not changing the information collected on the eFOIA screens.
