ADDENDUM TO THE SUPPORTING STATEMENT FOR FORM SSA-581
AUTHORIZATION TO OBTAIN EARNINGS DATA
FROM THE SOCIAL SECURITY ADMINISTRATION

We are making the following minor changes to form SSA-581:
Change: We substituted fillable boxes for blank lines, and added a barcode.
Justification: Our new version of the form will be scanned, and the information will be “lifted” and transmitted into SSA’s systems. This requires the use of fillable boxes and a barcode.
Change: We moved some information to different areas.
Justification: We did this to make the form more visually appealing and easier to read. The order of the questions remains the same.
