SUBJECT: Request for Approval to Extend Use of the Performance Assessment Tool to Collect Ambulatory Care Quality Measure Data
Attached please find an application under the Paperwork Reduction Act to continue to use the Performance Assessment Tool (PAT) for collection of clinical quality measure data for three Medicare demonstrations. The tool has previously been approved under two separate approvals for use in the Physician Group Practice (PGP) Demonstration and the Medicare Care Management Performance Demonstration (MCMP). The current approval for use of the tool under the MCMP Demonstration expires on December 31, 2008. The current approval for use of the tool for the PGP Demonstration expires on January 31, 2009. Because the tools are substantially the same, we are combining our request for extension for both demonstrations into one submission. Therefore, we will not separately ask for an extension of the PAT for the PGP Demonstration. Use of the Form ICR 0938-0942 titled: PGP Standardized Ambulatory Care Quality Measure Collection Initiative (CMS-10134) will be discontinued after its current approval expires January 31, 2009. Please refer to the attached document for further details.
Both the MCMP and PGP Demonstrations are congressionally mandated and will continue data collection past the current expiration dates and through FY 2011. In addition, in 2008 the Centers for Medicare & Medicaid Services will implement the Electronic Health Records (EHR) demonstration, a high Secretarial and Administration priority, to encourage the adoption of EHR’s in small to medium sized primary care practices. This demonstration will involve collection of the same clinical quality measures using the same data collection tool and methodology as the MCMP demonstration. The first year clinical data collection for this Demonstration will begin in the fall of 2011, and, therefore, is included under this request for the Paperwork Reduction Act approval.
