Dear Ms. Bryant,


The United States Trustee Program would like to modify appendix F to the debtor education provider application, OMB # 1105-0085. These changes are non-substantive formatting changes, and for the most part, mirror those made to appendix E to the credit counseling agency application, OMB # 1105-0084, which OMB approved on January 25, 2007. The changes are as follows:


  • Include contact person who can answer UTSP questions regarding appendix F

  • Include provider number

  • Require actual rather than estimated figures

  • Specify which six month period is covered by the appendix by adding text “Please submit this report within 15 calendar days following the end of each six-month period” at top of page.

  • Add text “Reporting Period (check one)” and screen fillable boxes marked “January-June” and “July-December” at top line inside border.

  • Reformat the appendix to present course evaluation summary data first and certificate data second

  • Eliminate subject matter sub-headings for each category of course evaluation data (course, teacher, course environment, course results)

  • Gather data on total number of evaluations completed for each type of delivery method (in-person, telephone, and internet)

  • Gather data on total number of students receiving instruction during reporting period

  • Eliminate data collection for “Other fees or charges collected from debtor students.”
