Major Changes since the first circulation:

The following changes were made at the request of TEGE.
Under General Instructions, we added the section Accounting Method.
Under Contributors to be Listed on Part I, we added a reference to
governmental units” as a contributor. Also, we added the section Contributions.
Under Section 501(c)(3) organizations that file Form 990 or Form 990-EZ, we
added clarification to the 33 1/3% support test.
Throughout Specific Instructions, we added numerous instructions for the
columns in Parts I and II.
