Researchers at the National Institutes of Health need your help. We are conducting a national survey of physicians regarding the care provided to cancer survivors who have completed active treatment for their disease. We have tried several times to contact you regarding your participation in this important study, but we have yet to receive a completed survey from you.


Your response is extremely important to us. You are part of a national sample of physicians, and your response represents thousands of other physicians in the United States like you. Without your information, the findings will not accurately represent what is currently occurring in physician practices across the United States. Accuracy is crucial; study findings will impact public health initiatives, medical student education, physician training, and other outcomes.


We have recently sent a survey package to you via Priority Mail. In appreciation for your participation, an honorarium check of $50 was enclosed in the package. Please support the National Institutes of Health and our Public Health Service by completing and returning the survey.

We previously sent a survey to you, but so far we have not received your completed questionnaire.  


You are part of a national sample of physicians, and your response represents thousands of other physicians in the United States like you.  If the study doesn’t include your information, the findings will not accurately represent what is currently occurring in physician practices across the United States.  Accuracy is crucial because study findings will impact funding for public health initiatives and training for physicians and medical students among other outcomes.  


Even if you believe that treatment of cancer survivors is a relatively unimportant part of your practice, your response is extremely important to us.  We are interested in how you practice – no matter what it is you do.
