Changes to P4P Evaluation Web-based Survey based on 061709 Conference Call

The following changes on the P4P Evaluation Web-based Survey were agreed to on the OMB/CMS/University of Colorado, Denver conference call held on 061709:

1. Delete the Directions statement for Questions 14 – 16.

2. Change the stem on Question 14 to:  What is your best estimate of the effects of the activities related to the P4P Demonstration will be on the cost of providing care to your patients?

3. Change the stem on Question 15 to:  How much of an impact on your patients do you think your quality improvement activities will have on the outcomes targeted in the P4P Demonstration?

4. On Question 16, change the 4^th column heading to read “Slightly Positive” and the 5^th column heading to read “Very Positive”.

NOTE: The fourth item was discovered while making the first three corrections.

As you will see in the accompanying version of the P4P Evaluation Web-based Survey (p4p_survey-final-061709), these changes have been made in the instrument. (We have some minor formatting issues that are being resolved to improve the appearance of the instrument.)

Based on the discussion during the conference call, we believe we have met all of the requests for changing the instrument requested by OMB.
