The estimated average burden associated with this information collection is 30 minutes per respondent. Burden
estimates include the time for reviewing instructions, gathering and maintaining data in the required form, and
completing the information collection, but exclude the time for compiling and maintaining business records in the
normal course of a respondent’s activities. A Federal agency may not conduct or sponsor, and an organization (or a
person) is not required to respond to a collection of information, unless it displays a currently valid OMB control
