Instructions
1. This form is to be used by licensees to report all transactions in which an unlicensed person acquired two or more pistols or revolvers or any combination of pistols or revolvers totaling two or more at one time or during five consecutive business days. This form is not required when the pistols or
revolvers are returned to the same person from whom they are received.
2. If the licensee has submitted a Report of Multiple Sale or Other Disposition of Pistols and Revolvers and thereafter, within 5 consecutive business days,
transfers an additional pistol(s) or revolver(s) to the same transferee then a new form must be submitted. When completing the new form, check the box
in line 2c and supply the date the previous form was submitted.
3. Item 2d - if the buyer of the firearm is a corporation, company, association, partnership or other such business entity, an officer authorized to act on
behalf of the business must complete Item 4 of this form with his or her personal information. Information relating to the corporation, company,
association, partnership or other such business entity should be placed in item 13.
4. A separate form is to be submitted for each unlicensed person.
5. Licensees must complete items 1 through 12 entirely. Items 13 through 17 must be completed, if applicable.
6. The report is to be submitted to:
a. Copy 1 - The National Tracing Center no later than the close of business on the day that the multiple sale or other disposition occurs. You can
either fax the form to 877-283-0288, or mail it to U.S. Department of Justice, Bureau of Alcohol, Tobacco, Firearms and Explosives, National
Tracing Center, P.O. Box 0279, Kearneysville, WV 25430-0279.
b. Copy 2 - The official designated by the State or local authorities to receive this form, or in the absence of such designation, to the department of
State police or State law enforcement agency in the jurisdiction where the handgun transfer occurred.
Note to the Recipient of Copy 2:
18 U.S.C. 923(g)(3)(B) provides in part that you certify in writing every six months that no disclosures have been made and that the multiple sales
forms on non-prohibited persons have been destroyed as required by law. The following statement can be used for this purpose. "I hereby certify
on behalf of (specify agency) that for a period of six months (specify dates) there have been no disclosures of Multiple Sales Forms contrary to the
provisions of the Brady Handgun Violence Protection Act, and that all forms and any record of the contents thereof have been destroyed as provided
by that law. Statement is to be dated and on agency letterhead, signed by the proper official and forwarded to the following address:

c. Copy 3 - Regulation 478.126a requires a licensee to retain a copy that should be attached to the back of the Firearms Transaction Record, ATF Form
4473, covering the transfer of the firearms.

Paperwork Reduction Act Notice
This request is in accordance with the Paperwork Reduction Act of 1995. The information collection documents certain sales or other dispositions of
handguns for law enforcement purposes. The information is used to determine if the buyer (transferee) is involved in a unlawful activity, or is a person
prohibited by law from obtaining firearms. The information requested is mandatory and required by statute (18 U.S.C. 923(g)).
The estimate average burden associated with this collection is 15 minutes per respondent or recordkeeper, depending on individual circumstances. Comments concerning the accuracy of this burden estimate and suggestions for reducing this burden should be directed to Reports Management Officer,
Document Services, Bureau of Alcohol, Tobacco, Firearms and Explosives, Washington, DC 20226.
An agency may not conduct or sponsor, and a person is not required to respond to, a collection of information unless it displays a currently valid OMB
control number. Confidentiality is not assured.
