Revised/eliminated tables for readability (Qs 2 & 12)
Updated boilerplate per DHS guidance (i.e., revisions to Qs 5, 7, 9, 10, 11, 13, 16, 17 & 18)
Updated cost burden by use of more recent Standard Rates instruction (i.e., revisions to Qs 12 & 14).
