Revision to Form 33 (From Voluntary to Require)
OSHA is proposing to require that all State Onsite Consultants (Consultants) use
the Revised Form 33 (revised in October, 2000) if they collect information in the
course of their consultation visit. There is no additional burden to the consultant.
By requiring the use of the form, data will be standardized and will ease analysis of
data. In addition, by requiring the form Consultants will have a valuable tool to
ensure all necessary safety and health programs elements are assessed.
