Individuals complete forms SS-5
(domestic residents) and SS-5-FS (U.S. nationals living abroad) to
request a new or replacement Social Security Number card. In this
ICR, we are making the following revisions to these forms: 1) we
are updating question #6, Race/Ethnic Description, to reflect the
correct OMB Race/Ethnicity categories and 2) we are making several
minor changes to the forms instructions. We are also implementing
a new internal-use system for recording SS-5 responses called
SSNAP, but this will not affect the publics experience in any way.
Note that the IC section represents an average completion time for
all application scenarios. Please see the supporting
statement(question #12) for the different completion times
different types of applicants may take.
