JUSTIFICATION

1513-0111 COLAs Online Access Request TTB F 5013.2

TTB is requesting approval to make changes to this form. The major change is we are deleting the Social Security Number (SSN) requested in box 7 and the reference to the SSN on the 2^nd page, in the “Privacy Act Information” paragraph. We are removing box 11 and redesignating boxes 12 through 22 as 11 through 21. Also, we are adding “/ADD PERMIT” after “MODIFY USER” in box 1.

These changes will not affect the burden hours because it doesn’t change the amount of time it would take to complete this form. The major change is the elimination of the SSN and in most cases that information would require the respondent only a moment to recall it. The time it would take not to include the SSN is minimal and therefore would not change the burden hours.
