As a result of the HEOA, changes to the currently approved application form under the Leveraging Educational Assistance Partnership and Special
LEAP programs are necessary. Specifically, 1) Item 10(a) and 10(b) under Section C need to be combined into one data element; 2) Language
needs to be altered in the instructions of #22 on by replacing the $5,000 reference (the change does not affect the data collected) and a similar
change on the second assurance on the last page. And 3) Added assurance on the last page about grants being LEAP Grants.
