Child Health USA 2008­2009 User Survey 
Introduction
In order to learn more about the needs of our databook users, we would like you to complete this
brief survey about your use of Child Health USA 2008—2009. The survey should take no more
than 10 minutes to complete. The survey is being conducted by the Maternal and Child Health
Information Resource Center, managed by Altarum Institute. You can complete this survey and
return it in the enclosed envelope, or complete it online at www.altarum.org/xxxxxxxx.
