Audience Group:

Thank you for agreeing to participate in this project. We are going to talk about organ donation. I am interested in everything you have to say about this topic, both positive and negative. There are no right or wrong answers. It’s helpful for me to hear everything you have to say, including knowing if you’ve never thought about some things before.

Do you have any questions about the informed consent form we sent you prior to the interview?

Do you have any other questions for me before we get started?

[Interviewer note: Use neutral prompts until answers are exhausted before moving to next question. Interview should not take longer than an hour.]

Neutral prompts:

  • Can you be more specific?

  • Anything else?

  1. If you had a friend or family member who was thinking about organ donation, would you show him or her this brochure? Why or why not?

Thank you so much. I am working for a government agency called the Health Resources and Services Administration. This agency encourages people to register to become organ donors. For more information, please visit organdonor.gov.

This completes our interview. I am required by law to report to you the OMB Control Number for this public information request. That number is XXX, which expires YYY.

Thank you again for your participation in the study!
