Dear [Name],


We, at the Centers for Disease Control and Prevention (CDC), invite you to take part in a new research study called the “Survey of Health Behaviors.” Several weeks ago, the California Cancer Registry (CCR) sent you a letter explaining a little bit about this study. A few weeks after that letter was sent, we sent you a copy of the survey. We have not heard back from you and would like to give you another chance to help us with our survey.


This study is being done to learn more about how men and women manage their health and daily lives. The survey asks questions about health habits, thoughts and feelings about healthy living, and things that may make it hard to live a healthy life after cancer. By taking the time to complete this survey, you will help us teach doctors, other health workers, and researchers about the ups and downs of trying to live a healthy life after cancer. The results also may be used for new programs to help cancer survivors and their families live longer, healthier lives after cancer.


It may be helpful to know that this study is approved by CDC and the State of California and is being done through a contract with Macro International Inc, the agency responsible for collecting data for this study. All information you provide will be kept private to the extent allowed by law. Nothing you tell us will be shared with anyone outside the research team.


We hope you will take part in this study. If you choose to take part, you will need to read the “Participant Consent Form” sent with this letter, and fill out the survey. Once you have finished the survey, please mail it back to us in the envelope provided. Please keep the pen sent with this letter.
