Changes to Questionnaire, August 5, 2009

The following changes have been made to the attached questionnaire. The question numbers have changed, and the explanations below refer to the new question numbers (as seen in the attachment).

1. Question 12

• Some items have been re-worded to be more general topics, rather than specific factual statements, because the question asks about topics (not statements).

• Items from original Q14 have been added to Q12 because the two questions were the same and share the same answer choices.

• Original Q14 therefore has been eliminated as a stand-alone question.

2. Question 14

   • “Public service announcements” has been replaced by “ads” because the latter is easier to understand.

3. Question 16

• The topics “How to answer your child’s tough questions about sex” and “Teens and mental health” have been added and “The benefits of marriage” has been deleted to better reflect the current contents of the 4parents.gov website.

• The wording of the item about risky behaviors of teens (fourth from last) has been changed slightly to improve its clarity.

4. Questions 18 and 22

• The response category, “Less than 1 minute,” has been added so that all possible responses are provided.

• The skip instruction attached to the “No” response to Q17 and Q20 has been deleted in those questions and moved to appear following Q18 and Q22. This corrects the skip instructions.

5. Questions 29, 30, and 31

• These three questions were originally asked as one question. The single question asked about three different types of materials. To clarify the question, we have broken it down into three questions, one for each type of material.

6. Questions 34M, 35M, 34F and 35F

• We have reversed the order of the two questions about puberty, to present the question that is easier to answer first.

• The words “groin area” in Q35M and Q35F have been replaced by “genital area” for better accuracy and clarity
