We are requesting approval to make two changes to this form. These changes will update one item and eliminate one unnecessary instruction.

The changes we are proposing is to, one, change “Taxpayer Identification Number” to “Employer Identification Number” in box three. We are also eliminating the second bullet in instruction A of the General Instructions.
