General Application Instructions for Small, Rural School Achievement Program (REAP)
NOTE: Eligibility for this program is restricted. Local educational agencies (school districts) that believe they may be eligible can apply; however, funding is conditional based on final eligibility determination.
In order to apply for this program, you will need to provide the following information:
· The legal name and address of your local educational agency (LEA; i.e., your district).
· Your LEA’s D-U-N-S number. If your agency does not have a D-U-N-S Number, you can obtain a number by calling 1-800-333-0505 or by completing a D-U-N-S Number Request Form. The form can be obtained via the Internet at the following URL: http://www.dnb.com/ .
· Your LEA’s Tax Identification Number (T-I-N).
· Your Local Educational Agency Identification Number in the National Center for Education Statistics’ (NCES) Common Core of Data (known as the NCES LEA ID). If you do not know this number, go to http://www.ed.gov/programs/reapsrsa/eligibility.html and locate your LEA in your State’s spreadsheet. (follow the links to FY 2008/School Year 2008-09, and then follow the links to the application process). If you cannot find your LEA there, you can also go to http://www.nces.ed.gov/ccdweb/school/index.asp and perform a keyword search for your State. If you are still unable to find your LEA’s NCES LEA ID number, please fill in the following code: 9999999.
· Whether your LEA is delinquent on any Federal debt.
· The name, address, phone number, fax number, and e-mail address of the person who will serve as the contact person for all matters related to this grant.
· The name, title, phone number, fax number, and e-mail address of the authorized representative of your LEA (i.e., your district’s superintendent). You will also be asked to submit a faxed copy of the authorized representative’s signature.
ELIGIBILITY
State educational agencies (SEAs) have previously been asked to make an initial determination concerning which LEAs in the State are eligible for funding under this program. To find out if your LEA is eligible, go to http://www.ed.gov/offices/OESE/reap.html and look at the Excel spreadsheet for your state.
