CBP Form 339C Changes

The revisions to page 1 – section 1 of the old form (equivalent to page 1 – section 1 of the new form), are the addition of the following fields:

• Optional physical address if different from shipping address.

• Contact e-mail address.

• Optional secondary contact (name, phone, fax, e-mail).

The revisions to page 1, sections 2, 3, and 4 of the old form (equivalent to page 1 – section 2 of the new form) are:

• The payment section has been significantly revised to make calculating total cost for order easier.

The following applicant responsibility language has been added to the bottom of each page of the new form: “Submitting this application certifies that all information is accurate. The applicant is responsible for ensuring that duplicate User Fees are not requested. All transactions are final. No refunds and no credits will be approved. To determine delivery date, add processing time and the shipping time for the options chosen.”

The new fields being requested on page 2 of the old form (also page 2 of the new form) are the following:

• Model

• Cab #

• Color

• License plate country code

• License plate state code

• Transponder ID field replaces decal number field.

• Indicating whether vehicle already has a transponder, and if yes indicating whether the listed number is correct.

• Indicating whether they are carrying for a C-TPAT FAST approved carrier, and if so what their FAST ID number is.

• Indicating whether the registered owner is C-TPAT FAST approved, and if so what their FAST ID number is, and if not they need to list their address.

Note that page 3 of the new form is identical to page 2 of the new form, so the same changes apply to page 3 as were discussed on page 2.
