Subj.: VA is requesting inclusion of revised VA form 10-5588, State Home Report and Statement of Federal Aid Claimed.
The updated version VA Form 10-5588 form was submitted in the proposed rule; however, the form was not provided to the ICR coordinator for inclusion in the approval. VHA inadvertently requested approval for an old version of VA Form 10-5588. Following OMB’s approval of associated rule making for 2900-AM97, there were two other revisions to the form due to a change in requirements by law for state homes operating beds that was just issued-- 38 CFR 51.43(c). VHA is requesting approval on the revised form. The minor revisions do not affect the burden hour.
