Survey and 24-Hour Recall

Dear Teacher:

You may wish to read this as you distribute the parental permission forms to encourage student participation in the National Youth Physical Activity and Nutrition Survey.

Our class has been selected to participate in the National Youth Physical Activity and Nutrition Survey conducted by the Centers for Disease Control and Prevention. The survey has two parts. The first part is a classroom survey that is designed to focus on physical activity and dietary behaviors that can cause problems during both youth and adulthood. The second part involves telephone interviews done outside of school that ask about the foods you eat. You can do up to three interviews and for each telephone interview, you will be given $10. If you complete all three interviews, you will be given an additional $10. Your participation in the survey and telephone interviews is very important as you represent thousands of students around the country. Each of you will receive a parental permission form to take home for a parent or guardian to sign. Please bring this form back within the next three days. You must return this form to participate. IF YOU DO NOT HAVE A SIGNED PARENTAL PERMISSION FORM, YOU MAY NOT PARTICIPATE.
