End of Season Report, 2008

Dear Reporter:

The information requested in this survey is needed to prepare the end of year report on acreage, production and prices received for fresh market vegetables this year. Response is voluntary and not required by law. However, your report is needed to make the estimates as accurate as possible even if you did not produce any vegetable crops this year.

Please complete and return this report in the enclosed envelope which does not need a stamp. Your individual report is confidential and used only with other reports for statistical purposes. Thanks for your cooperation.

Please make corrections to name, address and Zip Code, if necessary.

INSTRUCTIONS:

• Include all acreage on which commercial vegetables were grown for SALE.

• Exclude home use or home garden production and greenhouse production.

• If more than one vegetable crop was harvested from the same acres, report acres for each crop.

• If harvest is not yet complete for late season crops, please provide your best estimate for production & price data.

• If vegetables were planted in 2008, but failed or were NOT harvested, please report planted acres on the back page under the appropriate crop.
