Several States will conduct a production and disposition survey on certain specialty crops within their State. They will use this similar layout and insert the commodity of interest in the appropriate blanks, such as cabbage, or squash. Please make corrections to name, address, and Zip Code if necessary.			This survey is used to estimate the production and disposition of the 2009 __________ crop.
