Sheet 1: Instructions

SUPPLEMENTAL INSTRUCTIONS AND DEFINITIONS FORM:
General:
·         Do not include logs, bolts, or chips sold or transferred from your mill/plant to another mill/plant.

We want to eliminate double reporting of wood used in pulp and particleboard manufacturing.
·         If reporting for more than one mill/plant, please use a separate form for each mill/plant.


Page 1:
·         Fill in the name of the plant or company and its mailing address. Also, the name and

telephone number of a person that can be contacted if there are any questions concerning the data.
·         Under the section labeled "MILL TYPE/PROCESS" please check the box that best describes your mill.

Use "Other:_____" if your type of mill is not listed and print the type of mill in the space provided.
·         Under the title "Product produced", please check the box that best indicates the major product

the mill produces from the roundwood received for the year. To the right of the box is a list of

the primary products, if the mill produces anything other than what is on the list please check box

code 39 for pulp products, or 49 for particleboard products, and print the name of the major

product produced.
·         Fill in the amount in tons per day for pulp mills or hundreds square feet 3/4-inch basis for particleboard

plants of the average production at your plant after the label "Average production:_____."
·         The Note section can be used to comment on events that may have effected the mills production,

such as extended down time or mill expansions.


Page 2&3: Section I ROUNDWOOD AND CHIPS FROM ROUNDWOOD RECEIVED
·         In the section labeled "Check unit of measure" check the box that describes the scale used

to measure the wood at the mill yard.
·         Under the "Origin of Pulpwood" show the state and county or foreign country where

the timber was harvested.
·         Chips from roundwood are chips produced from logs, bolts, tops, limb wood, and whole trees

that have been completely chipped.
·         When whole tree chip receipts are recorded, please write (W) after the quantity.

When peeled roundwood receipts are recorded, please write (P) after the quantity.

(Debarked chips should be recorded as peeled roundwood - (P) )
·         Receipts of chips from roundwood should be recorded in section I, never in Section II. Receipt

of chips from mill residues (slabs, edgings, veneer cores, and other byproducts) are

recorded in Section II.

·         Check the appropriate box under "Unit of Measure".
·         The Origin of Byproducts is the state or foreign country where the mill producing the byproduct

is located. County of origin of byproducts does not need to be reported.
·         Receipts of mill residues or chipped mill residues (slabs, edgings, veneer cores, sawdust, and

other byproducts) should be recorded in Section II, never in Section I.
·         Do not record receipts of mill residues (slabs, edgings, veneer cores, sawdust, and

other byproducts) that are burned for fuel.
·         Chips receipts from chip-n-saw operations are recorded in Section II, if the chips came from

logs and bolts used to produce both lumber and chips as joint products. Here, the

chips are arbitrarily considered a byproduct.


Section III DISPOSITION OF BARK RESIDUE AND FINES
·         Enter your best estimate of the percentage of bark and fines that was used for the various

purposes indicated.
·         The percentages for each species of bark and fines should each add to 100%.
