Forest Industries Data Collection System

GENERAL INSTRUCTIONS

The objective of this survey is to collect and compile basic data on the source and volume of raw
materials used by the forest products industry in STATE and the volumes and types of finished wood
products produced in the state. Figures from individual firms will be held strictly confidential. Only
summarized data for the industry and for geographic areas will be made available. This report is
authorized by Public Law 98-378. While you are not required to respond, your cooperation is needed to
make the results of this study comprehensive, accurate, and timely.
Please fill out the attached questionnaire as completely and accurately as you can. Some of the
items may not apply to your firm's operations. In that case, please proceed to the next item.
