Revision to the Collection Instrument
We are making the following changes to the SSA-3288:
Page 1:
We are changing the language in the instructions for using and completing the SSA-3288 for clarification purposes.
We are changing the language in the Privacy Act Statement.to comply with the requirements of Privacy Act of 1974, as amended.
Page 2:
We are making minor wordsmithing and cosmetic changes to the SSA-3288 to assist respondents in making specific requests for information. These changes will save time for SSA field office and component employees who process requests for SSA records information.
We will destroy stocks of the old version of the SSA-3288 and start using the new version of the SSA-3288 immediately after OMB approval. We will also replace the current PDF available from SSA's website with the new version.
