Please report your potato stocks and prices received monthly. All individual reports are confidential and used only for statistical purposes. Response to this survey is voluntary and not required by law. However, your cooperation will provide useful information to your industry on the current potato stocks and price situation. Please review the instructions before preparing the report.

Please make corrections to name, address and Zip Code, if necessary.


INSTRUCTIONS

STOCKS:

Report total quantity of potatoes, regardless of ownership, or potential uses, on the first of April, less shrinkage and loss to date. Be sure to include potatoes held for seed. Report in hundredweight (cwt).

SHRINK and DUMP:

Report the projected shrinkage and dump amount, in hundredweight or as a percent of the total production, for the current season’s potato crop from March 1 through April 1.

SALES:

Only report sales of potatoes grown by you. Exclude potatoes purchased from other growers. If you did not sell any of your potatoes last month, enter “none”.
