The form CDC proposes to use is a
modified version of OMB 0970-0334 -Performance Progress Report
(SF-PPR) which was successfully piloted by the Administration for
Children and Families (ACF). CDC intends to use this modified form
as a quarterly uniform standard reporting format to be used by
grantees to meet the performance reporting requirements contained
in the terms and conditions of their Recovery Act funded Federal
Awards administered to States, Cities, Tribes, and Community
Coalitions by CDC. As a defined set of standard formats, the SF-PPR
facilitates the collection of Congressionally-mandated and needed
program-specific and agency-specific information by allowing
information to be collected uniformly. The form contains
identifying data elements and a section for a performance
narrative. Expedited review under AARA allowances to ensure
compliance for the October 2009 recipient reporting
period.
