Terms of Clearance


OMB approves this collection formerly in violation of PRA consistent with SSA

responses on 4/26/07 and 5/16/07. SSA will include in the section on representative payees, a clarification revised to say: "This information is ONLY needed if your provisional benefits will be sent to your prior representative payee."


SSA included the clarification above in the section on representative payees, as requested in the Terms of Clearance.


Revisions to the Collection Instrument


  • Per the Terms of Clearance, we included the clarification in the section on representative payees to read: “This information is ONLY needed if your provisional benefits will be sent to your prior representative payee.”


  • We are inserting a statement:

For persons who are entitled to any other SSA benefits based on disability or blindness:

I understand that if SSA denies my request for reinstatement because I have medically improved my current entitlement to SSA benefits will be reviewed and may terminate.”


    • We are adding this statement as a result of an approved employee suggestion. The new statement provides respondents with additional information so that they may make an informed choice to file for EXR.

    • We will start using the new version immediately after OMB approval;

    • The Field offices may use the old version until SSA releases the new version.


  • SSA's Office of the General Counsel is conducting a systematic review of SSA's Privacy Act Statements on agency forms. As a result, SSA is updating the Privacy Act Statement on this form.
