Your response is critically important to your local community and to your country. By answering these questions, you help provide local and national leaders with the information they need for planning schools, hospitals, and other community services. The information is also used to develop programs to reduce traffic congestion, provide job training, and plan for the healthcare needs of the elderly.

I asked you to help us with this very important survey by completing a questionnaire and mailing it back, but we have not received one yet. If you have already mailed a questionnaire back, thank you very much. If you have not, please complete one and send it now.

I have included another copy of the questionnaire with this letter. This survey is so important that a Census Bureau representative may contact you by personal visit if we do not receive your completed questionnaire.
