Subject:  Model Performance Evaluation Program for HIV Rapid Testing

Dear «MSALUTE» «MLASTNAM»:

Your laboratory has not submitted testing results for two or more survey periods in the Centers for Disease Control and Prevention (CDC) Model Performance Evaluation Program (MPEP) for HIV Rapid Testing.  We can only continue to send performance evaluation panels to laboratories who participate.  If we have not received an indication from you within two weeks of your desire to continue to participate in the MPEP, your laboratory will be removed from our enrollment records for this program.
