Purpose

The purpose of this communication is to: 1. explain details regarding the current Questionnaire Survey, the purpose of which is to collect information about HIV rapid testing practices, and  2. provide you with information regarding: the WEBSITE address for the MPEP how to submit your HIV Rapid Testing MPEP Questionnaire results online your MPEP PASSWORD the deadline for result submission Note: Results are now ONLY being accepted online- no paper forms to mail. MPEP is a non-regulatory quality assurance program

Your role

The results of the survey will support our mission of improving public health through continuous improvement of testing practices.  Participation in the survey is voluntary.  However, it is important for statistical validity that we collect as many responses as possible. How to submit results online

The MPEP now accepts results only online. In order to enter your results online, you will need: the current URL (website address) for online result entry (see above), your 5-digit MPEP number, which may contain leading zeros (0’s), and is noted above (first page, upper left) as part of your facility’s address, your facility’s online data entry password. Your password for entering your performance evaluation results online is: «Password».

SUBMISSION DEADLINE

Please complete & submit the enclosed survey form by: SEPTEMBER 19, 2010

Report of Results

You will receive a link to the online report of the aggregate results from all survey participants.  No individual results will be analyzed or reported.
