The SSA-263-U2, Waiver of Supplemental Security Income Payment Continuation, is currently in use without an OMB number. SSA discovered this inadvertent violation of the Paperwork Reduction Act in September 2009, and we compiled this clearance package to correct the lack of an OMB number.
SSA’s Office of Privacy and Disclosure created a new Privacy Act Statement for use with this form. In addition, upon approval of this ICR, SSA’s Forms Management Team will format the SSA‑263‑U2 so that it follows the same formatting as all SSA forms.
