Summary of Changes
Form 4506T-EZ is used by individual taxpayers in order to request a free tax return transcript of the 1040 series
returns. Taxpayers may request a transcript to be sent to a third party. The Spanish-language counterpart, Form
4506T-EZ(SP), mirrors the English-language form.
The purpose of this abbreviated form is to assist in the processing of mortgage applications under the "Home
Affordable Modification Program" (HAMP) and ease the process of obtaining tax return transcripts for taxpayers who
are in the process of either modifying or refinancing their mortgages.
An emergency OMB approval is requested due to Treasury's determination in releasing a new form immediately to
ensure that HAMP participants receive their correct tax information in a timely manner. Per Treasury's request, this
form must also be available in Spanish in order to assist the limited English proficient (LEP) population in gathering
their tax information as well.
