Questionnaire for Alumni Volunteer Involvement

 

VISTA is now accepting applications from VISTA alumni who are interested in recruiting new members and providing leadership to current members.

Time Burden: The time required to complete this collection of information is estimated to average 60 minutes per applicant. 

 

Privacy Act Statement

Authority:  42 U.S.C. §4953 of the  Domestic Volunteer Service Act of 1973, as amended, authorizes the collection of the requested information.

 

Principle Purpose: The information collected constitutes a questionnaire for VISTA alumni designed to create connections among alumni and engage them in recruiting new volunteers.

 

Routine Use:   Information provided for this collection may be shared with federal, state, and local agencies for law enforcement purposes.

 

Mandatory or Voluntary:  Participation in this information collection is voluntary.
