This collection is approved with the following terms of clearance: USAID is approved an emergency clearance for the collection in order to address the immediate disaster relief need. As a condition of this approval, USAID must publish a 60 day Federal Register Notice seeking comment on this collection within 14 days of this approval.

The purpose of this Information Collection is to enable USAID to collect information from members of the public as it relates to in-kind donations or expressions of interest to volunteer in response to an international disaster or emergency where USAID has been tasked with relief or recovery responsibilities.
The Disaster and Emergency Relief information request is needed as soon as possible and is essential to the USAID mission in Haiti. USAID has been tasked by the president to actively participate and coordinate relief and reconstruction efforts after the massive earthquake hit the island on January 13, 2010. The Agency cannot reasonably comply with the normal clearance procedures due to the urgency in delivering relief supplies and transporting emergency volunteers to the site. As such, massive public harm will result if the normal clearance procedures are followed. USAID requests that OMB provide us with clearance by February 19, 2010. This information collection is voluntary and only for those members of the public who 1) are offering in-kind donations or 2) are volunteering to assist with the relief efforts. The information that is being collected is the minimum amount necessary to ensure that the donations or volunteers meet minimum standards for fitness or skill.
