Dear Dr. Schwab:

Attached are two 10-day letters from the staff of the NCHS Questionnaire Design Research Laboratory (QDRL) (Generic clearance OMB 0920-0222, exp. 03/31/2013). Due to severe time constraints, we have submitted them together.

The first letter describes plans to conduct research to evaluate the Respiratory Health of Healthcare Workers Questionnaire for CDC’s National Institute of Occupational Safety and Health (NIOSH).

The second letter describes plans to conduct research to evaluate the Blood Donor History Questionnaire for the Office of the Assistant Secretary for Health (OASH).

The total number of burden hours for the two projects is 125 hours with 125 participants (125 participants at 1 hour each).
