Terms of Clearance

SSA complied with the terms of clearance; all Certification of Prisoner Identity Information templates include the OMB Control Number.

Revision to the Collection Instrument

Change: We revised the Certification of Prisoner Identity Information template by removing “Other Names Used by Inmate” and “Other Social Security Numbers.”

Justification: We recently updated the Memorandum of Understanding (MOU) agreements we enter into with prison facilities for the purpose of issuing replacement SSN cards to prisoners. The MOU specifically states that inmates who apply for a replacement SSN card under the terms of the MOU must already have a Social Security number and have never used a different Social Security number. Further, the prison facility must specify in the MOU established institution policy and procedures to verify the identity of every inmate in custody. The inmate’s name is verified by the official completing the template, so “Other names used” is not necessary for certification of identity. In addition, inmates shown to have used multiple Social Security numbers are not covered under the MOU and are not eligible to have their identity verified using the certification template.

We are revising the PRA statement to reflect our current boilerplate language. The current language, which dates back to the last reprint of the form, is now outdated.

SSA’s Office of the General Counsel is conducting a systematic review of SSA’s Privacy Act Statement on agency forms. As a result, SSA is updating the Privacy Act Statement on the first page of the form.
