The NA Form 13042, Request for Information Needed to Locate Medical Records, needed to be changed due to a facility move. Now the form reflects one address – NPRC, 9700 Page Ave., St. Louis, MO 63132. Another slight change was to the wording on the last sentence on page 2, item a. This changes was to clarify the need for requester’s to provide ‘proof of death’ when requesting documents on a deceased veteran. We also wanted to emphasize the need for legal guardians to provide copies of the court order proving guardianship.
