Green Folder – Materials to Fill Out and Return

Thank you for your interest in SEED. All of the materials in the Green Folder are for you to fill out and return. Please look for the following items in the Green Folder:

• Provider Check List: We are asking you to complete this checklist of medical providers for the biological mother and child.

• HIPPA (Health Insurance Portability and Accountability Act) Medical Records Release Form: The purpose of this form is to grant each of your health care providers permission to give the SEED study staff access to your medical records only for the purposes of the SEED study.
