Blue Folder – Cheek Swab Kit

Thank you for your interest in SEED. All of the materials in the Blue Folder are for you to complete and return. Please look for the following items in the Blue Folder:

• Cheek Swab Consent Form: The consent form gives you all of the information about you and your child’s cheek cell sample. It tells you who you can call if you have questions. Please review it before you complete the cheek swabs. One copy is for you to keep and the other copy is for you to sign and return in the enclosed envelope.

• Instruction on Collecting Cheek Swabs: This includes instructions on how to collect cheek swabs.

• Cheek Swabs that You Use. We ask you to complete these and return them in the enclosed mailer. This will take about 20 minutes to complete.
