If you wish to revise a previous report (such as if you report isolates as “influenza A, subtype unknown” and subsequently subtype the isolates), please indicate the changes on a copy of the form on which the isolates were initially reported, listing the original week ending date. Clearly mark the copy as a revised report and fax it to us.

October and May, a weekly updated influenza activity report is
available by facsimile 24 hours a day from the CDC Fax Information
Service at 888-CDC-FAXX (888-232-3299, request doc. #361100) or by
Internet at www.cdc.gov/flu/weekly/fluactivity.htm.
Public reporting burden of this collection of information is estimated to average 10 minutes per response, including the time for reviewing instructions, searching existing data sources, gathering and maintaining the data needed, and completing and reviewing the collection of information. An agency may not conduct or sponsor, and a person is not required to respond to a collection of information unless it displays a currently valid OMB control number.
