This evaluation was performed by Jim Thatcher and Kathleen Wahlbin
during May 2008. We evaluated the Survey Monkey end user survey form
only.
Criteria
1194.22(a) A text equivalent for every

non-text element shall be provided (e.g.,
via "alt", "longdesc", or in element content).
