We have made changes to this form that will make this form more useful and easier to follow. Specifically, we changed the format of the 1^st section from 2 columns to 1. The information itself has not changed. We have added an explanation for when to use the “User Information” box. In the box entitled “For Alcohol and Tobacco Companies only,” we have added information for permission to create, save, and submit reports and/or returns. Following that box, we have added a box to create, save, and reassign permission only. In the definition section we have changed the title to “Appendix 1” and added 2 new definitions (numbered 11. and 12.). Finally, we have made updates such as corrected the address for submitting this form and changed “Branch” to “Section” in the section entitled “TTB USE ONLY,” and we have made grammatical corrections.

With these changes, we believe that the time it takes to complete this form will remain at 5 minutes for each respondent.
