TTB is requesting approval to make the following changes to the above form:

1. Add the following statement after “Bond Execution” on page 1:

(By signing the bond, you acknowledge and agree to the Terms of This Bond

on page 3 of this form.)

2. Move “BY:” from “Principal/Obligor Name” to the next line

3. Insert “Execution” before “Date” at the top of page 2

4. Add a dollar sign at the top of each section under the “Operations Coverage”

column on page 2

5. Remove “Principal/Obligor” and “Date” from the top of page 3

6. Remove the “Acknowledge Terms Below” section at the bottom of page 3

7. Rewrote instruction 10.

We believe that these changes will make the form easier to complete.
