Appendix E

Instructions to develop the Unique Code


Thank you for taking this survey. By now you should have already read through and signed the assent form. We want to make every effort to protect your privacy and respect the confidentiality of your answers. We will now show you how to create your unique code. As you will see, it will be very easy for you to create but very hard for someone else to figure out. There is no need for you to memorize your code. Your code is made up of letters and numbers. Now here are the instructions. There are 7 spaces at the top of your survey.


  • In the first three spaces, enter the first three letters of your mother’s real name. For example, if your mother’s first name is Jennifer, enter the letter “JEN”.

  • In the next two spaces, enter the day of your birth. So, if your birthday is November 3rd you would write “03”

  • In the next two spaces, enter the first two letters of the name of your FIRST best friend. (YOU DO NOT HAVE TO BE FRIENDS NOW)


You have just created your unique code. Your unique code should have 5 letters and 2 numbers. If you feel that you have made a mistake or left a space blank because you did not know what to enter, please ask for help.
