SUBJECT: 0990-0317 - HHS Supplemental Form to the SF-424 (HHS 5161-1) – REVISION

DATE: August 30, 2010

HHS is renewing OMB No. 0990-0317, which includes the HHS 5161-1 form and three supplemental documents. We realize that the HHS 5161-1 form used in this request contains older versions of three forms. Upon approval for the continued use of this form HHS will use the most recently approved versions of 4040-0004, 0980-0204, and 0348-0046 in the final document provided to the public.
